I am Cambodian, so that I know much about Khmer.

I am trying to correct all the part in Wikipedia now. But it seems a bit complicated for me in some sections. Maybe I don't know much about its structure. Anyways, I am trying my best.

As it's a proper noun, so I don't dare translate it. But we do can translate it with many options: -ព្រះវិហារពិតនៃព្រះយេស៊ូ -ព្រះវិហារយេស៊ូគ្រិស្គពិត There are many other possibilities. But I can come up with these above two. As I am not a Christian and have very limited knowledge on it, so I can't help alot. I am a Buddhist, and what I can help is witht the translation only. Here is my translation: "The Holy Communion is the sacrament to commemorate the death of the Lord Jesus Christ. It enables us to partake of the flesh and blood of our Lord and to be in communion with Him so that we can have eternal life and be raised on the Last Day. This sacrament should be held as often as possible. Only one unleavened bread and grape juice should be used".
